Errors are often learning opportunities, so industrial companies have implemented, feedback systems (REX - retour d’expérience), for many years.
Although feedback is well established in companies, it still raises tensions: drift towards simple reporting, feedback conditions during organisational changes (outsourcing, specialisation, etc.)
This report offers an aid to reflection in the form of 40 questions to ask yourself concerning your own procedures and practices for feedback , informal sharing, capitalising on know-how, and organisational learning, such as:
- Does management support and encourage feedback?
- How to evaluate the effectiveness of corrective actions?
- Does information circulate well among different specialist areas?

About the author
This Cahier de la sécurité industrielle is the outcome of research by a working group led by the Foundation for an industrial safety culture (Foncsi). Over a period of two years, the members compared their experiences with the findings of 8 academic research projects on feedback. The working group met with representatives of Fédéchimie FO, EDF R&D, IRSN, Total, EDF, Areva, Sanofi, CFDT, GDF Suez, SNCF, Exxon-Mobil, and Eurodisney.
