  • The 2005 BP Texas disaster was one of the worst oil refinery accidents the CSB was asked to investigate. This four-minute video, made ten years later, in 2015, presents a brief review of the key messages, and provides useful feedback for the industry as a whole.

    The main CSB finding regarding this tragedy was that it was due to a poor safety culture and defective safety management, combined with badly-maintained equipment and regulations that were ineffective in promoting prevention.

    The questions addressed in the video include: What were the most useful recommendations? How can motivation be maintained in the long term, to achieve ongoing progress regarding safety? The answers to these questions will benefit workers, the public, and companies.
