Situations that could be very serious are an integral part of the design of installations and their procedures and, typically, three types of defence are put in place: prevention, recovery and mitigation. How do they work? How can we ensure that they are effective?
Moreover, once operations are up and running, unexpected situations can occur: for example, equipment failure, changes to production plans, or unexpected absences. These disruptive events can weaken the system’s defences, especially in combination. How can we identify the resulting risks and prepare appropriate responses?
